Criminal History Check Request

Investigators conducting background checks on members of the GW community must complete the online Criminal History Check Request Form, which is available below.

A member of the GWPD Criminal Investigations Division (CID) will be in touch with the investigator making the request in order to arrange an in-person appointment to release the results of the check.  An appointment will be made with GWPD CID beforehand to complete the necessary documents and conduct the check during a single in-person visit.

Investigators must present an official ID and a copy of the information release form signed by the subject of the criminal history check.
